Something we don't want to need, but still need to think about, is what if your child is injured or ill while away at college? If they are over 18, you will need certain forms to be able to make decisions for them or even get info from the hospital about them. There are paid versions of these forms, but here are free versions. You definitely need a HIPPA and medical power of attorney form on file before your child leaves. Keep a copy on your phone so you can easily access from anywhere. Personally, I don't care about having access to UGA Health Center info, so I did not ask my child to sign that. If they want mental or sexual health visits, I don't consider that my business. But if they are injured or ill and end up at the local hospital, that's when I want to be able to have information. On the Medical POA, there is a place for notary, but Georgia does not require a notary, just 2 witnesses.
